The Westwood Community School District allows the use-rental of facilities & Grounds to outside groups and organizations. The forms listed below are required to be completed in full, approved (Signaute) by the building Pricipal and submitted to the Operations Department at the District administrative Office a minimum of
10 Business Days prior to an event.
- Application for the Use of School Facility
- Please review your planned event with the building principal prior to submitting completed forms. The Pricipal must sign the form for it to be approved.
- Food Service Sales - Facilities Use Form
- Must be completed even if no food sales are planned.
- Sales of consumable food items must comply with all regulations established by the Wayne County Health Department.
- Certificate of Liability Insurance Form
- Naming the Westwood Community School District as also insured.
- Waiver of Liability Form
All request must be reviewed, approved and signed off on by the
building principal prior to submittal.
