Facility Use and Rental Forms - Instructions for use and application information
The Westwood Community School District allows the use-rental of facilities & Grounds to outside groups and organizations. The forms listed below are required to be completed and submitted to the Operations Department a minimum of
10 Business Days prior to an event.
- Application for the Use of School Facility
- Please review your planned event with the building principal prior to submitting completed forms.
- Food Service Sales - Facilities Use Form
- Must be completed even if no food sales are planned.
- Sales of consumable food items must comply with all regulations established by the Wayne County Health Department.
- Certificate of Liability Insurance Form
- Naming the Westwood Community School District as also insured.
- Waiver of Liability Form
All request must be reviewed, approved and signed off on by the
building principal prior to submittal.
